I (Parent/Guardian) am aware of my child's health and physical condition and having knowledge that their participation in any exercise and or basketball program may be injurious to their health, am volunteering to allow my child to participate in all physical activities. Having such knowledge, I hereby acknowledge this release, any representatives, agents and successors from liability for accidental injury or illness which they may incur as a result of participation in any physical activity. I hereby assume all risks connected therewith and consent to all participation in said program. I agree to disclose any physical limitations, disabilities, ailments and /or impairments which may affect their ability to participate in any physical activities acquired before and during any activities. * Required
